  • How does a health facility become a member of UPMB?

    LIST OF LOWER LEVEL HEALTH UNIT REGISTRATION REQUIREMENTS

    1. MoH Checklist
    2. UPMB Checklist
    3. DHO recommendation letter
    4. DHC (Diocesan Health Coordinator) recommendation letter
    5. Letter  of recommendation  from  Bishops or  Religious leaders of non COU at this level
    6. Picture (medical equipment, current staff, medical/theatre/lab/ administration/finance rooms
    7. MoH current operating License and NOT only supervising  doctor’s annual practicing license
    8. Proof of land ownership on which facility sits
    9. VISIT to the center by quality assurance officer, UPMB. (this visit expenses are met by facility)
    10. GPS
    11. ICT
